PLEASE CHECK BACK IN MARCH FOR INFORMATION ON THE 2012-2013 SCHOOL YEAR FORMS.

The following forms are required for the 2011-2012 school year.  Please complete and return by August 1st. 

NOTE: Medical forms are required as follows:

Physicals are required for ALL students NEW to the school (preschool through fifth grade) and for ALL KINDERGARTEN students.  Physicals done after August 31, 2010 are acceptable.

Dental exams are required for ALL students NEW to the school as well as ALL KINDERGARTEN AND THIRD GRADE STUDENTS.

TB forms are required for ALL students NEW to the school.
